To schedule an appointment, please provide us with the following information:

• Referral from your physician
• Your CSST file number
• Accident date
• The name of your CSST agent
• Your CSST agent's phone and fax number
• CSST Administration Form

Once we receive approval you will be contacted within 48 hours to schedule an appointment.
